Contracted through LA Net Community Health Resource Network, Toi serves as a Practice Transformation Coach. She supports clinics in delivering high-quality, evidence-based care through sustainable quality improvement. Her role includes technical assistance, data support, and workflow redesign to help practices implement patient-centered care models and meet compliance deliverables across HEDIS, CAHPS, and state quality programs.

Toi served as a live training facilitator for the Keeping Your Eyes Healthy initiative, a collaborative effort with the American Diabetes Association. She provided real-time education and support to Community Health Workers and Advisors (CHWs) in Birmingham, Alabama, helping increase awareness of diabetes-related eye disease and promoting strategies for prevention and early detection in high-risk populations. Contracted through LA Net.

Provided QI coaching and technical data support for the WPC program, serving Medi-Cal beneficiaries with complex needs including homelessness, mental illness, substance use disorders, and chronic disease. Toi helped clinics coordinate integrated care services, improve reporting, and align interventions with whole-person care principles.

Marketing Consultant and Coordinator to support the Medications for Addiction Treatment (MAT) Access Point initiative. Toi designed SharePoint content, developed patient facing educational materials, and coordinated outreach strategies targeting providers, patients, and community partners to promote awareness and utilization of MAT services with Los Angeles County Department of Health Services Behavior Health.

Contracted through LA Net to provide coaching and technical assistance for the CMS-funded TCPI program, in partnership with the Los Angeles Practice Transformation Network (LAPTN), LA Care, and LA County DHS. Toi’s scope included quality improvement coaching, academic detailing, data gap closure, patient and team engagement. Her leadership helped practices like H. Claude Hudson Clinic earn CMS recognition for Exemplary Practice performance.
